Hepatobiliary scintigraphy or abdominal ultrasound may be indicated if jaundice persists beyond two weeks in a term baby or four weeks in a preterm baby. Prolonged conjugated hyperbilirubinemia at that point raises suspicion for biliary atresia, a surgical emergency where the bile ducts are blocked. Early diagnosis — ideally before 60 days of life — dramatically improves outcomes with the Kasai procedure. This isn't common, but it's the reason we don't casually dismiss persistent jaundice.
